Mental Health Services Survey Dear Friends and Colleagues: Do you know something about adult mental health care services in your area?
If so, you are invited you to participate in a national survey of the process of transforming adult mental health care service systems toward the vision and principles of the President's New Freedom Commission (NFC) Final Report:
The survey asks you to identify one adult mental health care system in your state or community and to describe its current status on a number of indicators of recovery-oriented, and consumer and family driven systems. At the end of the survey you have some opportunity to provide comment. You can access the survey at http://www.contac.org/cgi-bin/Transformingwebsurv.pl The password is 62980. We estimate it will take no more than 15 minutes of your time to complete the survey. Please feel free to forward this message to others with knowledge about adult mental health care in your area. The survey is confidential. A copy of the final report of the findings of
this survey will be available from CONTAC later this year. We thank you in advance for your time and contribution to this valuable
study.
